Question: Angioplasty of the iliac with the sheath?

Can we bill 37220 for an angioplasty of the iliac artery with the sheath? I don't think so, but I just wanted to double check because the CPT description does not use the word "balloon".

Answer:
This is “Dotter” technique, and you are correct, if no balloon, no angioplasty. You could consider 37799 if a hemodynamically significant stenosis is treated with this technique. If it is only used to gain access to another vessel for a different intervention, I would not submit a code at all for this portion of a procedure.
